FLASHBACK: Officials at the crash site
The full detailed investigative report into the fatal microlight aircraft crash that occurred at the Tema Metropolitan Assembly (TMA) daycare school, claiming the lives of two is expected to be released on May 29, 2026.
This was disclosed by the Aircraft Accident and Incident Investigation and Prevention Bureau (AIB Ghana).
On March 16, some residents of Tema Community One, Site 18 were left traumatised following a microlight aircraft crash.
The victims were identified as victims, Captain Frank Amoaning Donkor and Elijah Ofori Donkor, both sons of Elder Frank Kwabena Donkor, leader of the Hebron Prayer Camp.
The investigator-in-charge of the case, Capt. Paul Forjoe, said while the eight-member committee—made up of five investigators and three observers—has completed the preliminary phase of the investigation, they await other inputs to complete the final report.
During the presentation of the preliminary report, Capt. Forjoe further stated that the investigation is waiting for the autopsy reports of the accident victims and needs to conduct further field investigations.
“Once we have all the requisite information and completed the necessary verifications, the investigation team will proceed to analyse all the information available to determine its conclusions. Which will include findings, causes which led to the accident and/or contributor factors which, if eliminated, avoided or absent, would have reduced the probabilities of the accident occurring, or mitigated the severity of the consequences of the accident,” he said.
Capt. Forjoe further stated that after analysing the needed inputs, “the investigations will then issue safety recommendations with a view to preventing reoccurrence. All the above will be captured in the investigation’s final report which we expect to be ready by Friday 29th, May 2026.”
He assured that the investigation team will continue to work diligently to release the final report promptly saying, “we hope that the end of the investigation, the final report will bring them closure and that the safety recommendations issued will prevent a re-occurrence of such an accident.”
At the press briefing, the Commissioner of AIB Ghana, John Wumborti, explained that the update was issued in line with both national law and International Civil Aviation Organization requirements.
“This is not the final report. We are satisfying the requirement of our law that says that after 30 days, if the final report is not ready, we should make the progress known to the public,” he said.
He stressed that the investigation was not intended to apportion blame but to identify lessons and propose safety recommendations to prevent future occurrences.
